What is Hope Home Care?

Hope Home Care is hospital quality care in your home. The multidisciplinary professional staff provides expert care that once was available only in hospitals or clinics. They also supervise certified home health aides who can provide assistance with bathing and personal care. If you require rehabilitative care, a physical, occupational, and/or speech therapist will work closely with you and your family to ensure you will make continued progress day after day.

Who is Eligible?

You must be under a doctor's care and need skilled services on an intermittent rather than a continuous basis. Any qualifying resident of any age, in Fulton County, Pulaski County, Marshall County, Miami County, Wabash County, Cass County, or Kosciusko County, is eligible to receive care provided by Hope Home Care.

Hope Home Care allows the patient to remain at home with family and familiar surroundings. By working with your physician, the Hope Home Care staff can teach you and your family how to manage your medical needs within your own home.

Hope Home Care may also help you prevent costly hospital admissions. It enables earlier discharges from hospitals, rehabilitation centers and long-term care facilities. We believe there is no place like home when recovering from injury, illness, or following a surgical procedure. Medicare, Medicaid, Veterans Benefits, and other state and federally funded programs, as well as private insurances, may pay for visits on your behalf.
